University Health Services

All students planning to enter in the fall semester are required to submit a medical history and physical report form to University Health Services by July 1st.

If you will be a minor when entering in the fall semester, your parents must fill out a consent form for you to be treated by University Health Services.

Indiana law requires that all incoming freshmen receive information about meningitis. You should have received information in your admissions packet. You will also find information at
http://www.cdc.gov/meningococcal/

You can enter your vaccination records online.

Entering your vaccinations requires a login with your NetID and password. If you have any problems with the online form, please contact Connie Morrow at 574-631-1882 or cmorrow@nd.edu.

The University offers the option of student health insurance. More information can be found at http://uhs.nd.edu/insurance-billing/.

Students who have private insurance will receive an itemized statement to submit to their insurance carriers. Some UHS services may not be covered. More information can be found at http://uhs.nd.edu/insurance-billing/.

The Health Service site and forms will be available to new students who have obtained their NetIDs beginning June 1.
